Free formal green employee training record template

This formal green employee training record template gives hospitality employers a structured history of one worker's learning. The supplied example focuses on Responsible Beverage Service topics and completion status.

Employee Information and Training Details organize the current record. A six-row Training History then lists the Date, Training Title / Subject, Trainer, Duration and Status for each activity.

Import approved employee data through Google Sheets or select another source from Certifier's integrations. After review, issue the training history through a verifiable credentials workflow so authorized reviewers can inspect its live record.

The employee training record can accompany a related compliance training certificate when both documents refer to the same program. The record preserves multiple training activities, while the certificate provides a shorter completion statement.

What is an employee training record?

An employee training record is an employer- or provider-controlled history of workplace learning completed by one person. It records when each activity occurred, who delivered it and the status entered by the issuer.

For responsible beverage service or hospitality compliance programs, the document can support an employee training certificate workflow. A Completed status does not by itself prove licensing, legal eligibility or job authorization where separate requirements apply.

The supplied employee document is designed for standard A4 dimensions (21 x 29.7 cm) and also works in US Letter format (8.5 x 11 in). Portrait and landscape versions are available. All you need is a free Certifier account to customize, edit and issue the document.

What does this formal green employee training record include?

A useful employee training record should reflect the hospitality employer's approved learning history. The following elements are visible in the supplied formal green design:

Static layout elements

  • Provider header and record controls: Fixed positions hold the SafeFleet Transportation Services logo, HR & Compliance label and contact email. Training Record ID and Issue Date appear beside the Employee Training Record title.
  • Employee Information and Training Details: Labeled spaces organize Employee Name, Employee ID, Department, Job Title / Position, Manager / Supervisor and Training Year. A parallel section provides the current training-event labels.
  • Training History table: The structured history uses Date, Training Title / Subject, Trainer, Duration and Status headings. Six visible rows cover ID authentication, intoxication recognition, refusal, documentation and patron safety.
  • Signatures and verification footer: Reserved lines support Employee Signature and Trainer / Supervisor Signature with dates and printed details. Credential ID, Training Record ID, Provider and Verify at labels complete the document.

Dynamic fields (automatically populated after mapping in Certifier)

  • Employee and workplace identity: Employee Name and Employee ID identify the record holder. Department, Job Title / Position, Manager / Supervisor and Training Year place the history in its employment context.
  • Training Details: Training Title / Subject and Trainer / Presenter identify the current activity. Venue / Workplace, Date & Time, Duration and Training Category explain where, when and how it was recorded.
  • Training History rows: Each row stores the Date and Training Title / Subject, such as Responsible Refusal & De-escalation. Trainer, Duration and Status show who delivered the activity, its recorded length and its outcome.
  • Authorization and verification data: Employee and Trainer / Supervisor signatures document review. Issue Date, Credential ID, Training Record ID, Provider and Verify at URL connect the document to its issued record.

These changing fields are visible in the supplied employee training record and are not guaranteed to be pre-mapped. Configure them in Certifier’s design builder and use only workplace learning data verified by the employer or approved provider.

How to issue the formal green employee training record

Use this four-step controlled workflow:

  1. Customize the design and your branding — Open the green record in the online editor. Choose A4 or US Letter and select portrait or landscape. Add the provider identity, then adjust the Training Details and responsible beverage service history rows.
  2. Bundle it with a matching companion document — Pair the record with the certificate it supports from the certificate template gallery. Check that Employee Name, Training Title / Subject and Issue Date agree across both designs.
  3. Connect your tools — Sync employee rosters and training results from the approved HR, compliance or learning system. Map Employee ID, Trainer, Duration and Status instead of re-entering those values for every worker.
  4. Issue, deliver, and verify — Review all six Training History rows and collect the required employee and supervisor signatures. Issue the approved credential set and deliver it to the employee. Open the Verify at URL before publication.

Who issues employee training records?

Hospitality employers and venue managers

Employers maintain training histories for staff who serve or supervise alcohol service. Venue managers may check dates and completion status. The record should follow the employer's approved compliance process.

Approved responsible beverage service providers

Training providers document the subjects, trainers and duration supported by their source records. They may prepare completion data for an employer. Provider approval does not replace any separate licensing decision.

HR, compliance and workplace learning teams

HR teams coordinate employee identity data and record retention. Compliance or learning administrators may prepare batches and collect signatures. The authorized issuer remains responsible for final publication.
