Professional sky blue healthcare provider BLS certificate template
This free healthcare provider BLS certificate confirms that a clinician completed your BLS course at provider level, covering adult, child, and infant CPR, AED operation, and airway management. It's designed for healthcare training academies that teach doctors, nurses, and allied health staff and award CPD hours for the course.
The page sits on a sky blue background with two white panels. The top panel holds your logo, a two-line uppercase serif title in navy, and a small QR code in the corner. The recipient's name runs across a sky blue band between two thin blue rules, with folded ribbon corners in a darker blue.
The lower panel holds the course statement and a row of four fields: course level, date completed, renewal due, and CPD hours. Signature lines for a medical training director and a course instructor sit at the bottom, with the credential ID centered between them.
After each course, issue certificates automatically through a native integration when a learner is marked complete in your booking or learning system, or upload a spreadsheet.
What is a healthcare provider BLS certificate?
A healthcare provider BLS certificate shows that a clinician completed basic life support training at the level expected in clinical settings, on a specific date. It also names the academy that taught it. Academies issue it as the course record, often with CPD hours the clinician can log.
The design says the training follows "current resuscitation guidelines," so name which council's guidelines in the course statement. A certificate from your academy records your course, but it isn't a network-issued card.
This template is laid out on A4 landscape (29.7 x 21 cm). Sign up for a free Certifier account, add your guideline wording and issue BLS certificates as soon as the course ends.
What should a healthcare provider BLS certificate include?
A clinician's BLS certificate should show the course level, the guideline followed, the hours earned, and when to renew. Use the online builder to edit these fields.

- Certificate title: The award name (e.g., Certificate of Basic Life Support).
- Signature and role areas: Two signature lines (e.g., Dr. Renata Voss, Medical Training Director, and James Whitfield, RN, Course Instructor).

- Course level: The audience level of the course (e.g., Healthcare Provider).
- Date completed: The day the course ended (e.g., January 22, 2026).

- CPD hours: Hours the clinician can log (e.g., 8 Hours).
Clinicians may also want a professional registration number on the page. Rename, add, or remove fields in Certifier's design builder.
How to issue healthcare provider BLS certificates
- Name your guideline: Edit the course statement to name the resuscitation guideline your course follows. Add your logo and both signatures.
- Connect your booking or learning system: Use Zapier or a native integration to send completions to Certifier, or upload a spreadsheet with names, levels, dates, and CPD hours.
- Issue with a renewal date: Keep the 2-year renewal or set your own expiration date (Professional and Advancedplan feature). Each clinician receives the certificate by email.
- Help clinicians log CPD: Clinicians download the PDF or share the verification link with their employer or for their CPD portfolio.

Frequently asked questions
Yes, you can customize this healthcare provider template on Certifier's free Starter plan, which allows up to 5 custom attributes. Its custom fields (course statement, course level, and CPD hours) fit within that limit. The QR code needs the Professional or Advanced plan, and so does the renewal date, which uses Certifier's expiry feature.
No. AHA cards are issued only through AHA-authorized Training Centers, and this template records a BLS course your academy delivers. If a clinician's employer asks for an AHA card specifically, the clinician needs to complete an AHA course.
Yes, if it does, and name which ones, for example the guidelines of the council your curriculum is built on. The issuing organization is responsible for that statement, not the template.
On this template, CPD hours is a field you fill in for each learner. Whether those hours count toward a clinician's CPD requirement depends on their professional body.
Yes. The course level is a field, so the same design can say Healthcare Provider for one class and a different level for another.
