Simple vine green trauma-informed care certificate template
This free trauma-informed care certificate records that a social worker or student completed your certificate program and how many contact hours they earned. It suits university schools of social work and their continuing education offices, which often run trauma-informed care as a multi-session program for practitioners in the field.
The layout is calm and centered on a soft cream background with a thin sage border. Leafy green vines with small gold dots run along the top and bottom edges. A shield crest with a leaf sits above the university name, and the title is set in a large dark green serif.
When a cohort finishes, import the class list from a spreadsheet or connect your LMS and issue every certificate at once. Each learner receives a credential page with a verification link on every plan.
What is a trauma-informed care certificate for social workers?
A trauma-informed care certificate for social workers is a record that a social worker completed a trauma-informed care program, with the contact hours earned. Many social workers submit these hours toward license renewal, so the certificate has to carry the details their board or approval body expects.
Only approved providers can award accredited CE. A school without that approval can still give each learner a certificate of completion that lists their contact hours, as long as it doesn't call them accredited CE.
If your school is an approved ASWB ACE provider, the certificate needs:
- The participant's license number
- The course format and location
- The amount and type of credit
- Your ACE provider number
- The ACE approval statement, in the prescribed wording
The certificate records completion of your program. It doesn't show that a social worker meets every renewal requirement in their state, which depends on their board.

What should a trauma-informed care certificate for social workers include?
Think about what the practitioner will hand to their licensing board, then make sure it's on the page.

How to issue trauma-informed care certificates to a social work cohort
- Set up the program design: Add your crest, signatures, and the approval statement. Add fields for license number, format, and location if your approval body asks for them.
- Export the cohort list: Pull names, emails, license numbers, and hours from your registration system or LMS into a spreadsheet. Upload up to 1,000 learners at once.
- Match columns and issue: Map each column to a certificate field and preview a few certificates. Then send them, and each learner gets an email with their certificate and verification link.
- Answer verification requests: When a board or employer asks for proof, the learner shares their credential page. You can also look up any certificate in your Credential Repository.
